  • How do I make an appointment?

    For an appointment please call 850-626-3760.

  • Do you take walk-ins?

    Appointment slots are kept open each day for acute problems, but calling ahead will reduce any delays on your part.

  • What are your hours of operation?

    Operating hours 8:00am-4:00pm.

    Extended hours 7:30am-8:00am and 4:00pm-5:00pm.

  • Do I have to pay for my visit at the time of the appointment?

    Copays are required at the time of your visit.

  • What will be my co-pay?

    Co-pays will vary according to your insurance plans.

  • What payments are accepted at the office?

    Cash, Check, Visa, Mastercard, Discover, and American Express.

  • What do I do if I have an HMO plan and want to change the doctor to your practice?

    Call your insurance company and request the doctor change.

  • What documents should I bring for my visit?

    Completed new patient information forms found on this website.

  • Can I leave a message for my doctor?

    Messages are taken by the operators and forwarded to the physician. Calls are returned during lunch and at the end of the work day.

  • What do I do if it is an emergency?

    You may call the office for instructions on how to handle your emergency or go directly to the Emergency Room.

  • Will my doctor see me if I am admitted to the hospital?

    Our Doctors make weekly rounds at Santa Rosa Medical Center. The hospialist round on the weekends.

  • Can I get medication refills over the phone?

    Request for medication refills are taken by the operators or you may call your pharmacy to make a refill request.

  • Can I have X-rays/ Blood tests performed at your office?

    Basic X-rays are performed in the office. The Santa Rosa Medical Center and Quest provide lab services on campus.

  • How can I get the results of my test?

    Results are reviewed within an 8-10 day period. You will be notified by phone or mail.

  • Who should I contact if I get sick after hours or weekends?

    You call the main office number. An answering service will contact the physician on call.

  • How do I get referrals for specialists?

    We have a referral department that handles all requests for appointments to outside facilties.

  • Is there a fee for filling out forms/letters?

    Yes.

  • Does the doctor fill out the form for mobility devices like scooters?

    Yes.

  • Will you bill my insurance company if my visit is for an auto accident?

    Yes.

  • Who do I talk to if I have a billing questions?

    There are billing/collections staff on site.

  • Is your office accessible for patients with physical impairments?

    Yes.
